Southwest Georgia Health Care provides primary health care services through 18 centers with 35 physicians, nurse-practitioners, physician assistants, and a superb team of nurses and other support staff. Twelve of our offices have been recognized by NCQA as PCMH (Patient Centered Medical Home) Level 3 practices.
