Were improving health in neighborhoods where better care is needed most, and to do that, we extend care beyond the traditional doctors office. Our care teams are based in the communities they serve, forging long-term relationships with members and partnering with local community-based organizations. Equipped with custom-built technology, we deliver personalized primary care, behavioral health care, and social services to each member.

Equipped with Commons, our care teams can seamlessly communicate and coordinate care, whether at a Cityblock Neighborhood Hub or in the community

And our product and engineering teams are constantly refining our technology to support care teams in what they do bestcaring for our members.

Cityblock was founded on the premise that health starts at the neighborhood level, and that everyone should have good care for what matters to them. We saw firsthand how healthcare has fallen short in providing adequate care for people living with complex health and social needsand knew there was room for improvement.
We aimed to build a new kind of care model that addresses the root causes of health, and in doing so, improves health in communities that have previously been underserved. Today, were delivering exactly that kind of care to thousands of members in New York and Massachusetts, & Connecticutand were not stopping there.
