Welcome to Tri-County! Established in 1965, Tri-County Community Action Agency offers a broad range of programs and services to provide a helping hand to low and moderate-income individuals and families.
As a community action agency, we are one of less than 100 agencies nation-wide that operate a Federally Qualified Health Center (FQHC). The Tri-County Health Center locations serve Rhode Island residents with the following programs: complete primary medical care for all ages, prenatal care, mental health and substance use counseling and treatment, integrated behavioral health, LGBTQ+ health program, Title X services, chronic disease management, dental & oral health and so much more!
We are currently recruiting for primary care physicians, pediatricians, and nurse practitioners. About the Community
Tri-County Health Centers are located in Johnston and North Providence, RI. Our providers typically have 1 main location and see patients out of the second location 1 to 2 sessions per week.
Tri-County Health Centers serve the towns of Johnston, North Providence and Smithfield, but we accept patients from all over Rhode Island.
