Job Description

The Nurse Practitioner works as part of the clinic provider team performing a variety of direct patient services including assessment, diagnosis, implementation of treat, evaluation, and monitoring the general patient management plan for the clinic patients. The Nurse Practitioner performs these functions in accordance with the collaborative practice agreement (CPA) with the collaborating physician in the primary health care setting.

Qualifications:
Must have a current unencumbered Louisiana APRN license. Must have graduated from a nationally accredited nurse practitioner program. Must have national nurse practitioner certification in area of specialty and sufficient experience to carry out the duties of this position. Basic Life Support Card required.

Responsibilities:
Obtains complete medical history from patient, and/or family, and/or previous medical record, etc. Performs complete physical examinations on patients. Assesses, diagnoses, implements treatment, evaluates, and monitors patients' health status. Orders routine and diagnostic lab and x-ray studies according to CPA. Diagnoses acute and chronic illness on basis of clinical findings, laboratory and x-ray. Report and/or results of other diagnostic procedures according to approved protocols. Formulates, initiates and monitors patient management plans for patients assigned to the Nurse Practitioner, using protocols and/or consultation with a collaborating physician. Prescribes medications according to protocols and therapeutic goals and explains, treatment to patients and families. Refers patients requiring hospitalization or assessment and management of conditions to collaborating physicians or appropriate specialist. Follow patient's progress with collaborating physician. Documents all care provided on the EHR in a timely manner. Renders emergency care including cardiopulmonary resuscitation, starting IV fluids and medications according to protocols.
Performs other necessary duties as required by PHSC to meet the goal of providing primary health care services.
