Job Description

The ambulatory care clinical pharmacist provides patient-centered medication therapy management in collaboration with other health care professionals. These services include providing drug information and patient education as well as providing collaborative medication therapy management in the ambulatory setting to improve clinical outcomes, prevent adverse drug events, and improve patients' quality of life. The pharmacist in this position serves as a key member of the interdisciplinary patient care team and promotes high quality, evidence-based, cost-effective pharmaceutical care.

Minimum Requirements

- Education BS in Pharmacy/Pharm.D.
- Experience ASHP PGY1 Residency in Ambulatory Care or 3 years of direct patient care and medication management for ambulatory patients.

Required Licenses/Registrations/Certifications

- Current license (or eligible) to practice pharmacy in S.C.
- Professional certification in applicable specialty area (ex. BCPS, CDE)

Core Job Responsibilities

- Provides patient care through the provision of direct clinical patient services and clinical pharmacy care (which may include point of care testing).
- Conduct comprehensive assessments of patients' health and medication histories to determine individual medication-related needs
- Utilize the standards of comprehensive medication management services which include assessment of patients' current medication regimens for indication, effectiveness, safety, and adherence
- Identify potential and actual drug-related problems, while resolving actual drug-related problems and preventing potential drug-related problems.
- Complies with appropriate regulatory agencies requirements, e.g. SC BOP, DHEC, DEA, etc.
- Follows attendance, dress, education (including continuing education) and safety polices
- Assists in the training and orientation of new employees; Precept IPPE and APPE Institutional students.
- Completes documentation of medication errors and near- misses, and handles appropriate communication with the patient and prescriber
- Develop and enter into collaborative practice agreements (CPAs), and manage patients medications for chronic disease states via CPAs, where applicable
- In collaboration with other members of the health care team, develop a care plan that includes individualized medication therapy goals, personalized interventions, and appropriate follow-up and monitoring to reduce disease burden.
- Provide drug information to patients, their families, and other health care professionals. Provide patient education regarding medications, disease states, and self-management skills
- Serve as a patient advocate to help coordinate care and discuss any potential or actual medication-related problems with the patient's physician and other members of the care team as appropriate
- Document all clinical and any research activities in compliance with established policies and procedures; must demonstrate accuracy, thoroughness, and attention to detail
- Other responsibilities as assigned

Location: Spartanburg Regional Healthcare System Specialty Pharmacy Admin
Schedule: Part-Time, Days, 8am-4:30pm M-F
