Job Description
The Stroke Nurse Practitioner is a registered professional nurse who uses specialized skills in the care of stroke patients and families. The Nurse Practitioner will be responsible for completing comprehensive patient health evaluations in collaboration and consultation with the Neurologists and the Neurosurgeons. They will assess an coordinate the educational and psychosocial needs of the patient and family, review the plan of care with bedside nurse and coordinate discharge planning in collaboration with the Case Manager and Social Worker.

Jersey Shore University Medical Center is home to a state Certified Comprehensive Stroke Rescue Center. Join a team of Stroke experts to care for stroke patients across a continuum of care. The Nurse Practitioner role for this Service line adds dimension for care across the stroke continuum from admission to discharge and re-intergration into the community.

As the Nurse practitioner, you will be responsible for management and treatment of the acute patient with emergent response to the Emergency dept.; work with an interdisciplinary team to expedite care; educate and assure individualized quality care is provided to all stroke patients. You will work collaboratively with emergency physicians and staff; neurologists and neurosurgeons to manage comprehensive and individualized care for stroke patients and families in the inpatient phase of hospitalization.
Manages comprehensive and individualized care through the inpatient and immediate post-discharge phase of the patients hospitalization experience
Utilizes teaching/learning theory and appropriate teaching strategies in the provision of education to nursing staff, hospital staff, patients and families, and the community.
Promotes an environment that facilitates learning by responding to appropriate patient/family preferences, priorities and readiness to learn.
Evaluates the effectiveness of patient/family teaching and collaborates with the nursing staff in the completion of education plan
Stroke Program Performance Improvement Plan.

Minimum of 3 years recent clinical experience as a registered nurse preferably in a Neuroscience Unit in the Hospital setting.
Masters degree in nursing required. Completion of Nurse Practitioner curriculum with successful completion of NP certification exam and minimum of 1 years hospital experience.
Licensure to practice as a professional registered nurse in New Jersey.
Certification in Neuroscience Nursing preferred
