We are seeking a full-time Director of Hospice/Palliative Care who will work with current directors of inpatient and outpatient palliative care programs to coordinate and develop a system-wide approach to palliative and hospice care. Candidate will report to the Valley Medical Group Chief Medical Officer and be responsible for coordinating all relevant discharges with the Hospitalist team/population health department, increasing continuity of palliative care through developing a network of post-acute and SNF facilities, coordinating palliative care and hospice services offered through the Cancer Center and other chronic care management service lines and work to coordinate services with primary care physicians, geriatricians, physiatrists, and others as appropriate. Must maintain Board Certification in Palliative Care and Hospice Medicine and have more than two years of experience in clinical care.
