DUTIES The Community Living Center (CLC)/Palliative Care team physician is accountable to the Geriatrics and Extended Care Line leadership. The Geriatrics and Extended Care, Hospice and Palliative Medicine physician will practice within their scope of practice in accordance with their credentialing and privileging. Duties include, but are not limited to: Provide physician services consistent with hospice and palliative care philosophy Consult with primary provider as requested so that pain and symptom management for patients is effectively and expeditiously accomplished Provide inpatient hospice and palliative care to Veterans admitted to CLC Hospice and Palliative Care beds Provide inpatient (acute medical wards, CLC) and outpatient consultation services in palliative care Be an active participant and facilitate clarification and the development of patient and family goals of care Facilitate access to appropriate supportive care services Full and appropriate documentation of patients care services to support professional billing for these services Weekly participation in interdisciplinary team meetings to review active cases Develop and provide Hospice and Palliative care education to Chillicothe VA staff and coworkers. May also be asked to participate in student or resident education efforts Adhere to the practice of confidentiality regarding patients, families, staff, and the organization Be involved in analysis of data leading to the development of hospital policies and protocols for the improvement of quality of care Analyze workload data and work with teams to maximize productivity Monitors and evaluates the quality of medical services as an integral part of the overall quality assessments and improvement program and assuring compliance with Joint Commission and Long Term Care Institute (LTCI) requirements Serves as member of the Geriatrics and Extended care medical staff, attending meetings, helping to assure adherence to its bylaws and rules and regulations - Performs other duties as assigned Work Schedule: Monday - Friday, 7:30am - 4:00pm or 8:00 am - 4:30 pm Statement of Duties Title: Hospice and Palliative Medicine Physician TRAVEL REQUIRED - Not Required RELOCATION AUTHORIZED - No
