DUTIES The Community Living Center (CLC)/Palliative Care team physician is accountable to the Geriatrics and Extended Care Line leadership. The Geriatrics and Extended Care, Hospice and Palliative Medicine physician will practice within their scope of practice in accordance with their credentialing and privileging. Duties include, but are not limited to: Provide physician services consistent with hospice and palliative care philosophy Consult with primary provider as requested so that pain and symptom management for patients is effectively and expeditiously accomplished Provide inpatient hospice and palliative care to Veterans admitted to CLC Hospice and Palliative Care beds Provide inpatient (acute medical wards, CLC) and outpatient consultation services in palliative care Be an active participant and facilitate clarification and the development of patient and family goals of care Facilitate access to appropriate supportive care services Full and appropriate documentation of patients care services to support professional billing for these services Weekly participation in interdisciplinary team meetings to review active cases Develop and provide Hospice and Palliative care education to Chillicothe VA staff and coworkers.
