Hospice Care Network, a Joint Commission accredited not-for-profit healthcare organization, seeks enthusiastic physicians to join a respected Hospice and Palliative care program in Nassau, Queens and Suffolk Counties.  Practice comprehensive end-of-life care, in a high quality and professional environment.  Our integrated electronic clinical records system assures superior access to clinical information across the continuum of care.

Job Responsibilities:

• Conduct home and face-to-face visits
• Collaboration with hospice interdisciplinary teams
• Provide expert symptom management
• Weeknight and weekend coverage on an on-call schedule
• Direct patient care each day

Qualifications:

• Board Certified/Eligible either in Hospice, Palliative Care, Internal Medicine, Family Practice, Geriatrics or Osteopathic Medicine
• Licensed to practice in New York State
• Fellowship training in hospice, palliative care or geriatrics is a plus
• Clinical expertise in caring for hospice or palliative patients and their families
• Excellent collaboration, communication and primary care medical skills
• Intermediate computer knowledge
