A faculty position has opened at Baylor College of Medicine -Texas Children's Hospital Transition Medicine Clinic (TMC) for a physician who is interested in outpatient primary care. The clinic is devoted to caring for adolescents and young adult patients transitioning to adult healthcare who have significant disabilities or chronic illnesses with many of the patients having an intellectual disability. We are looking for either a physician just graduating from Internal Medicine, Med-Peds or Family Community Medicine training program or is a seasoned primary care physician interested in practicing in an academic environment. Compensation will depend on experience. About Our Clinic: The TMC provides a medical home to our patients while providing expertise in identifying and treating disease-specific symptoms and conditions. The TMC is uniquely equipped to care for older adolescents and adults with chronic conditions of childhood. Our population consists of patients with Down syndrome, Spina Bifida, Cerebral Palsy, genetic disorders, congenital heart diseases and autism. Our clinic seeks to support the whole family as they navigate new systems of care and who face unanticipated challenges making the transition to adult healthcare. Essential Functions and Duties: Provide medical care to adolescents/young adults with chronic conditions of childhood. Participate in daily clinic session and supervise internal medicine and Med-Peds residents and medical students. Manage the health of the clinic population through care coordination and collaborating with case managers. Participate in weekly team meetings. Attend daily team huddle prior to clinic. Go to various case management meetings with our pediatric partners. Work with team to develop educational curriculum for healthcare students/physicians.
