About the Organization

Park DuValle Community Health Center, Inc. (PDCHC) is the oldest Section 330e federally qualified health center (FQHC) in the Commonwealth of Kentucky providing primary care for all ages including Adult, Pediatric and Womens Health, Behavioral Health, Dental, Laboratory, X-ray and Pharmacy services. We accept most insurances and eligible low income, uninsured patients are seen on a sliding fee scale regardless of ability to pay.

Park DuValle is an independent not-for-profit community health center and is a Level 3 NCQA recognized Patient Centered Medical Home. We have served Louisville and surrounding counties health care needs since 1968.

The center's services are affordable, comprehensive and of high quality. As a federally qualified health center surveyed and licensed in the Commonwealth of Kentucky as primary care centers, Park DuValle Community Health Centers four sites strive to adhere to the highest quality standards of patient care, regardless of insurance or ability to pay. Employees of Park DuValle Community Health Center, Inc. are deemed to be federal employees qualified for protection under the Federal Tort Claims Act (FTCA).

We are all about caring for you and our community and want our community to get healthy, be healthy and stay healthy. In collaboration with community agencies and universities, we offer free Breastfeeding, Smoking Cessation, Healthy Journey for Two Prenatal, and Active Steps for Diabetes health education classes. Low cost and free Mammography is offered in partnership with the Norton cancer Institute.

About the Community

Introduction to Louisville
Louisville, Kentucky is the largest city in the state and the sixteenth largest in the country. Louisville has 90 other attractions, 17,000 hotel rooms, and 2,500 restaurants to satisfy visitors and residents alike. The city was founded in 1778 by George Rogers Clark and named after King Louis XVI. It lies on the Ohio River near the border with Indiana. There are many charming neighborhoods in the city, including Old Louisville with its 19th-century charm.
