About the Organization
Horizon Health Care is made up of a network of community health centers with 32 Horizon clinic locations across the state of South Dakota. We are a primary care provider that has been nationally recognized by the Health Resources and Services Administration. In our rural communities, our health centers support a communitys ability to retain local health care options, supporting access to health care where rural South Dakotans live and work.
As community health centers, we develop special relationships with our patients. Each Horizon clinic is proud to offer quality health care regardless of cost. For patients who do have insurance, we can work with your insurance company and offer a sliding fee program if you qualify.
